The ONLY Acceptable Forms of Identification must have the same name or
LONGER than the name as it appears on the document to be signed, and
are as follows:
The Identification must be current, or have been issued within five years
(photocopies are not acceptable - must be original):
The following forms of Identification shall either be current or have been issuedwithin five years and shall contain a photograph and description of the person
named on it, shall be signed by the person, shall bear a serial or other identifying
number:
Credible witnesses (2 people who personally know the signer and have
acceptable ID) may be used when signer does not possess an acceptable form of
identification.

| A passport issued by the Department of State of the United States |

| A driver’s license issued by another state, or by a Canadian or Mexican | |

| A United States military identification card with the required photograph, | |
| description, signature and identifying number | ||
| An inmate identification card issued by the Department of Corrections and | |
| Rehabilitation, if the inmate is in custody | ||
| An employee identification card issued by an agency or office of the State | |
| of California, or by an agency or office of a city or county in this state |
